You do not need a referral from a health professional to participate in our service!
To join the Cockburn Healthy Lifestyle Service, all new clients must first attend an initial health consultation before our health consultants can enroll them into any programs. Health consultations can take 45 mins to an hour.

Health Consult Appointment Availability
- Monday: 9 am – 4 pm
- Tuesday: 11 am to 6 pm
- Wednesday: 8:30 am to 12:30 pm
- Thursday: 11 am to 6 pm
- Friday: 9 am to 4 pm
Updates to our clinic location
We have moved downstairs!
CHLS health consults will be conducted at Suite 8, 11 Wentworth Parade starting Monday 15th September 2025. We are on the ground floor, opposite Success Library and adjacent to Centrelink and Medicare.
We are located in the same space as Black Swan Health, Cockburn Wellbeing and Pregnancy to Parenthood.
